What is Chronic Condition Management
When patients face long-term diseases, our goal is to reduce the effects of the disease so that you still live your best lives. By understanding each patient as a whole, this service is designed to prevent, address, and monitor symptoms and disease progression to have you enjoy an elaborate life after knowledge. We take the guesswork out by scheduling routine check-ups, coordinating and adjusting treatment, ordering and/or interpreting lab results, and educating in simple terms to empower you and your loved ones. This hassle-free, customized approach provides consistent care yet still gives you the knowledge and tools to take back control of your life. Examples of Chronic Conditions include – Heart Diseases, Diabetes, High Blood Pressure, Thyroid Diseases, Asthma, Irritable Bowel Syndrome and more.
What is Medication Therapy Management?
Patients often face unique and tough challenges. Unlike an infection where treatment may be a week of inexpensive antibiotics, chronic conditions with or without acute health problems, may force patients to balance ongoing health and non-health related costs, compromise on other medications, and balance repetitive treatment into their lives. This service not only considers our extensive knowledge of medications, but also combines that with our understanding of you, your lifestyle, and your goals. We work with you and your physician to build a plan that addresses these needs, costs, compliance, with minimal interruption into your life. With this information, we feel that you are better informed with the most effective and cost considerable therapy options and have better treatment outcomes.
What does Medication Consultation Mean and Include?
When picking up a prescription, you’ll often notice the thick documentation attached to it. Reading through the small text almost requires a medical dictionary, a magnifying glass, and a lot of patience. This service is your ticket to skip the line and learn about your medications at the comfort of your home and your own time convenience in more than the traditional 30 second consultation. It is like having a pharmacist on speed-dial, ready to explain the benefits and side effects of the medications that your treatment may require. Patients who have a full understanding of a particular medication, have higher success rates of completing treatment.
What is Disease-Related Lifestlye and Behavioral Changes?
Our health and wellbeing aren’t just based on our family health history; it includes current factors such as lifestyle, mindset, and environment. The foods that we eat – did we eat pizza or fruit for lunch; the activities in our day – did we take the stairs or elevator; The daily stress we face – did we get enough rest and sleep; the choices we make are all important contributors. Understanding what these factors can possibly lead to, as well as finding alternatives, helps promote a healthier life. Our coaching provides you with the knowledge to identify these behaviors in your daily life, provides accountability for simple change and insight and/or approaches of daily organizational techniques to fit your health needs. It is designed with you and your loved ones in mind to help prevent disease as well as manage chronic conditions.
What is Preventative Medicine?
Preventative medicine focuses on proactive healthcare through personalized, holistic approaches. It emphasizes understanding and addressing underlying health issues before they become serious, integrating physical, mental, and emotional well-being. ELAK advocates for personalized prevention plans based on individual risk factors and lifestyle choices, aiming to empower individuals in managing their health effectively over the long term.
What are the Most Common Causes for Preventative Hospitalizations?
- 1% to 10% of hospital admissions are due to nonadherence to medications in the outpatient setting and are therefore preventable
- Non-adherence in the US accounts for 50% of treatment failure, around 125,000 deaths, and up to 25% of hospitalizations each year.
- Non-adherence to prescribed treatment remains a significant barrier to improved patient outcomes for CVDs.
- Nonadherence to medications has been documented to occur in > 60% of cardiovascular patients
- 24% of patients do not pick up their CVD medications after hospital discharge
- 34% of patients stopped at least 1 medication and 12% stopped all medications within 1 month of hospital discharge

Why Am I Frustrated with my Current Healthcare System & Delivery?
You are not alone. This is not about you. We are all experiencing the impact of decades of complications and imbalance between supply and demand. The reason for the current healthcare crisis is multifactorial:
- In the last decade, people with chronic conditions grew from 15 million to 133 million patients
- There is a shortage of thousands of primary care physicians and staff in the country which increased our current physician staff burn out to 41% for the added administrative and clinical burdens
- The average number of weeks for a patient to see their primary care physician in California is 2 to 3 weeks and in the country is 3 to 4 weeks.
- Research shows the average length of visit for the patient and the physician is 15.7 minutes and in those 15.7 minutes the doctor addresses the biggest problem in the first five minutes and then one problem per minute afterwards.
- At the pharmacy, patients wait in line for 30 minutes and then eventually see the pharmacist for 30 seconds.
- The pharmacist may ask: “What questions do you have for me”. Logically the patient cannot know what they don’t know and now they go home with medications they barely know about.
- Patients are left on their own to gather knowledge from family, friends, internet resources.
- Patients afterwards are in noncompliance and nonadherence to medicine use, which is the number one root cause for hospital visits, ED and urgent care visits in the country.
- 34% of Americans do not pick up their medications upon discharge from the hospital.
- 24% of patients stop one of their medications within the first month of hospital discharge.
- 12% stop all their medications within three months of hospital discharge. And the cycle repeats
